Ask before running anything new

The first time Hearth wants to run a command it has not run before, it asks. You can allow it once, always for this project, or not at all.

The permission prompt now names the exact command, the folder it will run in and why Hearth wants to run it. Pressing y allows it once. Pressing a allows it for this project and writes the rule into hearth.toml so your team shares it.

Reading is always allowed

Reading files inside the project never asks. Reading outside the project folder always does.
